Please read the following statements and place a circle around a number (0,1,2,3) that
indicates to what degree this statement has occurred to you during the past 2 weeks. The
rating scale is as follows:
0: Has not happened to me.
1: It has happened to me little or part of the time.
2: It has happened to me quite a bit or for part of the time.
3: It has happened to me a lot or most of the time.
