1. In terms of a standard health care setting, how would you define “Quality”?
In health care setting, quality defines the provided health care services for

2. On what particular aspect of health care does an accurate, reliable and timely result
play in the management of human disease/s.
I think it is the laboratory results since it must be accurate, reliable and timely.
3. What are the costs of laboratory errors?
The costs of laboratory errors are the patient outcomes, personnel effort and
time.
4. What are the essential elements of a Quality Management System? Describe briefly
each one of them.
I think these are the following essential elements of quality management
system:
Organization to have an organized structure and management of the
laboratory.
Personnel must be competent and motivated workers.
Equipment used in laboratory and must work properly as well as it is
maintained and managed appropriately.
Purchasing and Inventory manages the quantification of reagents and
supplies, and all are ensured to be in a good quality and available any time.
Process Control ensures the laboratory testing process and procedures
maintained its quality and condition.
Information Management manages the information and data of patients
which must be accurate and confidential.
Documents and Records must be maintained accessible, accurate and
confidential.

Facilities and Safety the workspace is provided with proper equipment and environment, and ensures the safety and security of the personnel and sample are attained.

The components of pre-analytical examination phase of laboratory testing are the following: - Patient Preparation - Sample Collection - Personnel Competency Test Evaluations - Sample Receipt and Accessioning - Sample Transport
  2. What are the components of the analytical phase of laboratory testing? The component of the analytical phase of laboratory testing is the quality control testing.
  3. What are the components of post-examination phase of laboratory testing? The components of the post-analytical phase of laboratory testing are record keeping and reporting.

where it all includes the entire process from patient to reporting and in between the process which errors and interruptions are avoided.

  1. What are the factors that influences a laboratory test? Laboratory tests are influenced by these factors: laboratory environment, knowledgeable and competent staff, reagents and equipment, quality control, communications, process management, occurrence management, and record keeping.
  2. What are the variables that plays an interactive role in the implementation of a quality policy in health care? The communication, responsibilities of authorities, and provision of resources are the variables that keeps the implementation quality policy in health care to be interactive.
  3. What can you do to keep your laboratory a growing environment in terms of personnel management? In order the lab to have growing environment in terms of personnel management, I think the staffs/personnel must be competent and motivated. Other than that, the lab should have orientation and training programs for them and professional development.
  4. Who is the one responsible for the maintenance, calibration, troubleshooting and record keeping of laboratory equipment in any level of a health care institution? Explains as broad as possible. I think the laboratory director is the one who generally supervises all the laboratory equipment management systems in any level of a health care institution. He/she ensures that the staffs/personnel are trained to use the laboratory instruments and equipment as well as they acquired proper knowledge to necessarily operate and perform routine maintenance procedure. In addition, laboratory director ensures to monitor the equipment management activities. Specifically, inside the laboratory, the medical technician/technologist are the ones who is responsible for maintenance, calibration, trouble shooting and record keeping of lab equipment.

Part II. 10 points each. Explain the following based on your own understanding of the concepts: a. Purchasing and inventory It is very essential component of the quality management system of a healthcare institution. It ensures that there is an availability of reagents and supplies as well as to prevent its wastage. Likewise, it certainly develops record-keeping system for receiving, inspecting and storing of supplies. And, it effectively handles the inventory system. b. Process control It is also an important component of quality management system because it ensures that there is a quality control for laboratory testing procedure. On the top of that, it appropriately handles the sample management together with its collection and method/procedure validation and verification. c. Information management Information management clearly manages the accuracy and confidentiality of the patient’s information/data on the documents and records. It also provides timeliness, accessibility, privacy and security of data towards the patient and laboratory staff. d. Occurrence management It is practically established to identify potential errors or mistakes that might happen and prevent them from happening. When error or problem is detected, there will be immediate investigation and corrective action to conduct by the management. e.
