Arterial Deficits - ANSWER- - dry, shiny skin
- skin cool/cold in tenmperature
- nails thickened
- no hair on toes and dorsum foot
- ulcers on toes, heel, or pressure pulse areas
- very painful ulcers
- edema minimal unless foot in DEPENDENT position
- pain intermittment claudication to sharp
Venous Deficits - ANSWER- - skin thickened & tough
- skin is reddish/blue
- ulcers on medial malleolus or anterior tibial
- superficial ulcer can be very painful
- irregular border

  • sensory perception
  • moisture
  • activity
  • nutrition
  • mobility
  • friction Stages of Pressure Ulcers - ANSWER- Stage 1 - redness, but skin is still intact; can be painful, firm or soft and may be a different temp than surrounding tissue Stage 2 - Partial thickness loss- very shallow, blister may still be intact but filled with serum; can be shiny or dry IE - tapeburn Stage 3 - Full thickness tissue loss not able to see bone, tendon or muscle; can be sluffing -falling off or tunneling Stage 4 - Full thickness tissue loss with exposed bone, tendon or muscle Skin Cancer - ANSWER- "ABCDE" A symmetry Borders Color Diameter >6mm (pencil eraser) Evolution or elevation Clubbing - ANSWER- Sign of hypoxia; could indicate pulmonary disease; common in COPD Jaundice - ANSWER- Yellowing of skin or eyes
  • dark patient --> assess SCLERA

Plaque - ANSWER- elevated, palpable, solid mass with a circumscribed border that is 1 cm in diameter ` Kussmaul - ANSWER- Labored, rapid, deep breathing, diabetic keioacidosis
